Ambitions for Palliative and End of life Care: A National Framework for Local Action 2015 – 2020
In partnership with 25 other national organisations we have developed the ‘ambitions framework’. This national framework provides a structured approach to deliver sustainable transformation at a local level.
At the heart of the framework are six ambitions:
Each of these ambitions is delivered through a number of building blocks. An example of this is ‘Each person is seen as an individual’, the building blocks to achieve this include ‘care which is integrated across health and social care’ and people having ‘honest and well-informed conversations about dying, death and bereavement.

For CCG’s the benefits of these ambitions and the foundations for the ambitions are not only within end of life care but across urgent and emergency care and long term conditions.
In order to deliver your STP’s we can work alongside your commissioning teams to integrate the ‘ambitions framework’ into your local strategy for end of life care.
The cost of this service is set out below:
- £480 per day (including VAT) plus expenses
- Discount for NCPC subscribers: £400 per day (including VAT) plus expenses
