This popular guide aims to explain advance care planning to the public. It outlines the different options available to people when planning for their end of life care.

Planning for your future care, which was originially published in 2009 with the National End of Life Care Programme, the Dying Matters coalition and the University of Nottingham, and revised in 2012. This new edition has been updated and revised for 2014, serves as an easy-to-read introduction to advance care planning, and is one of the most requested booklets at NCPC.
As you will know the National End of Life Care Programme was disbanded in 2013, so the ownership of Planning for your future care has transferred to NHS Improving Quality.
